 Can't reproduce #4, neither with the given OS/compiler/boost version, nor
 with other versions.

 Can't reproduce #5, neither with the given OS/compiler/boost version, nor
 with other versions.  Could be that neither postgresql-client nor sqlite3
 where available, and thus the logical_file adaptor did not build at all.
 The config / build logs are not included in the report.

 Can't reproduce #6, neither with the given OS/compiler/boost version, nor
 with other versions.  Yes, the example may be dubious, but is valid.  no
 idea why out.fail() would be reporting incorrect stream state.

 #7, #8: moved to separate tickets
